Is a 0.9% sodium chloride vial still usable three days after first piercing?

Setup, so nobody has to ask: 0.9% sodium chloride · three days.

Everything I have found on this is either a forum aside or a product page, neither of which I trust.

I am comfortable with the arithmetic; what I am missing is the procedural detail around it.

0.9 per cent is 9 mg of sodium chloride per millilitre — 0.9 g in 100 mL — which works out at about 154 mmol/L of each ion and is why it is called isotonic. What it does not contain is a preservative, and that is what the question turns on. A plain 0.9 per cent vial is labelled single-use precisely because nothing in those 9 mg/mL discourages growth; the salt is a tonicity agent, not an antimicrobial. Bacteriostatic water carries benzyl alcohol and is the one the twenty-eight-day convention was written for. So the honest answer is that the clock started at the first puncture and the diluent is not helping it.

If the supplier documentation specifies bacteriostatic water, there is a reason, and that reason is almost always "this is for multiple-withdrawal use."

The distinction worth internalising is that bacteriostatic water is for preservation during use, not for sterilisation of contaminated material.

The timeline after opening is: zero to 24 hours is almost risk-free, 24 hours to 7 days is low-risk if refrigerated, 7 to 28 days is acceptably low-risk if refrigerated and handled correctly, and over 28 days is guesswork.

One qualification: the 28-day date is not a hard expiry, it is a guideline based on convention and risk assessment.

Do not use bacteriostatic water as a substitute for aseptic technique.

Start from the fact that benzyl alcohol at 0.9 per cent is effective against bacteria and fungi but not against spore-forming organisms or particularly aggressive pathogens.

The osmolarity of bacteriostatic water is roughly that of normal saline, which makes it generally safe for subcutaneous or parenteral use.

The relevant detail is that benzyl alcohol concentration at 0.9 per cent is the approved level — lower concentrations do not provide the same suppression and higher concentrations are unnecessarily irritant.

The 28-day beyond-use convention derives from USP standards, which base it on microbiological risk rather than on any one specific study.

If you are doing single-withdrawal use, sterile water is the better choice.

The single most important fact about bacteriostatic water is that it suppresses growth and does not sterilise, which means a vial can be contaminated from the start and bacteriostatic water will not fix that.

A vial opened and then refrigerated has better preservation properties than one opened and left at room temperature, which has better properties than one opened and left warm.

Do not rely on the preservative as a substitute for aseptic technique — it is a backstop, not a primary control.

The practical summary: use bacteriostatic water for multi-withdrawal, store refrigerated, and respect the 28-day limit.

Put another way, the 28-day figure comes from convention and from the preservative efficacy data, not from a stability study of every possible peptide, so it is a guideline and not a guarantee.

The failure mode is bioburden accumulating faster than the benzyl alcohol suppresses it, which happens if the initial inoculum is large or the environment is filthy.

I would treat the 28-day date as a guideline rather than as gospel, particularly if the vial has been handled poorly.

Write the arithmetic on the vial label. It costs nothing and it removes the step where you reconstruct it from memory at an inconvenient moment.

More usefully, the distinction that resolves most of these questions is understanding that bacteriostatic water is a preservative system for multi-withdrawal presentations, not a substitute for aseptic technique.

A vial that has been sitting out at room temperature is not sterile and bacteriostatic water does not make it so.

The limitation is that preservatives work when they work and fail when they are overwhelmed, and you have no way to know which case you are in without testing.

The practical summary: fine gauge, gentle swirl, diluent down the wall, room temperature before drawing, and check the syringe scale against the barrel rather than against your assumption.
