Does holding at 1.7 mg for six weeks before escalating reduce dizziness?

What I have: 1.7 mg · six weeks · dizziness.

I suspect the usual explanation for this is wrong, or at least incomplete.

I am aware this may have a boring answer. I would still like the boring answer stated clearly.

Can someone derive this rather than assert it?

six weeks at 1.7 mg is 42 days at one dose level — and the arithmetic the question hides is that it also moves every dose above 1.7 mg back by 42 days. Whether that reduces dizziness depends on which quantity it tracks. A symptom driven by the dose itself is unchanged by waiting: 1.7 mg is 1.7 mg on day 1 and on day 42. A symptom driven by the rate of change has 42 days of no change to settle in, which is the case the hold is actually made for. The distinction is testable on your own record: plot dizziness against days-since-last-increase rather than against dose, and if the peaks line up with the increases the hold is doing something. Escalation schedules are set by a prescriber, and nothing here is medical advice.

Answer first: the titration schedule exists to let tolerance to the gastrointestinal effects develop between steps, and every step in it is a tolerability decision rather than an efficacy one.

For an agent with a one-week half-life, steady state after a dose change is reached in about four to five weeks. Escalating at two weeks means escalating from a position you have not yet reached, and the resulting exposure is higher than the schedule intends.

Liraglutide titrates weekly rather than four-weekly because its half-life is about thirteen hours, so steady state is reached within days. The interval follows the pharmacokinetics in both cases.

Trial data show gastrointestinal adverse events concentrated in the escalation phase and declining at stable doses.

The caveat is that titration decisions belong with a clinician who knows what else is on board.

Four half-lives between steps, minimum. Work it out for your agent.

The short version: four-weekly steps for the weekly agents, hold rather than escalate when symptoms are active, and slower is always available.

The published tirzepatide schedule steps 2.5, 5, 7.5, 10, 12.5 and 15 mg at four-week intervals, with 2.5 mg as the initiation step rather than a therapeutic one.

It helps to be literal here: the dose that produces an acceptable result with acceptable tolerability is the endpoint, not the top of the schedule. A substantial fraction of trial participants did well below the maximum.

The relevant arithmetic is that steady state takes four to five half-lives, so a one-week half-life means a four-week step interval and nothing shorter is informative.

Holding a step for eight weeks instead of four is a legitimate approach with no efficacy cost at the destination, and it is what the trials effectively did for anyone who tolerated poorly.

The relevant detail is that titrating downward is available too. Stepping back and holding is a normal adjustment rather than a failure.

Four to five half-lives to steady state is standard first-order pharmacokinetics and gives the four-week interval directly from the one-week half-life.

Hold rather than escalate while symptoms are active. Always.
