What is the case for and against 0.9% sodium chloride over two weeks of withdrawals?

What I am working with: 0.9% sodium chloride · two weeks.

I suspect the honest answer is that it depends, in which case I would like to know on what.

Assume I can obtain either option without difficulty, so availability is not the deciding factor.

Is there a defensible reason to prefer one, or is this a coin flip?

The failure mode is bioburden accumulating faster than the benzyl alcohol suppresses it, which happens if the initial inoculum is large or the environment is filthy.

Units are U-100 insulin units, where 1 unit = 0.01 mL. Divide dose by concentration for millilitres, then multiply by 100.

The underlying point is that the additive suppresses Gram-positive bacteria, Gram-negative bacteria and fungi quite effectively, but it is not an adequate control for spore-forming organisms.

The 28-day beyond-use convention derives from USP standards, which base it on microbiological risk rather than on any one specific study.

If you are doing single-withdrawal use, sterile water is the better choice.

Start from the fact that benzyl alcohol at 0.9 per cent is effective against bacteria and fungi but not against spore-forming organisms or particularly aggressive pathogens.

The timeline after opening is: zero to 24 hours is almost risk-free, 24 hours to 7 days is low-risk if refrigerated, 7 to 28 days is acceptably low-risk if refrigerated and handled correctly, and over 28 days is guesswork.

The relevant detail is that do not rely on the preservative as a substitute for aseptic technique — it is a backstop, not a primary control.

One qualification: the 28-day date is not a hard expiry, it is a guideline based on convention and risk assessment.

The practical summary: use bacteriostatic water for multi-withdrawal, store refrigerated, and respect the 28-day limit.

Benzyl alcohol sensitivity is real and worth checking — some people react to it on injection and some peptides are destabilised by it.

The osmolarity of bacteriostatic water is roughly that of normal saline, which makes it generally safe for subcutaneous or parenteral use.

Mechanically, a vial opened and then refrigerated has better preservation properties than one opened and left at room temperature, which has better properties than one opened and left warm.

Published pharmacopeial standards specify the use and limits of benzyl alcohol in preserved presentations.

The caveat is that bacteriostatic water is not sterile water for injection — they are different products with different purposes.

Do not use bacteriostatic water as a substitute for aseptic technique.

It helps to be literal here: the short answer is that bacteriostatic water buys you time in a multi-withdrawal vial — roughly 28 days refrigerated by convention — but it does not make a non-sterile presentation sterile.

Different manufacturers of "bacteriostatic water" vary in their formulation, and some are better than others — brand matters.

Benzyl alcohol efficacy in suppressing common skin and environmental bacteria is well-established and published.

If you are doing single-withdrawal use, sterile water is the better choice.

A bacteriostatic water vial, once pierced, is conventionally treated as good for 28 days refrigerated, and that is the limit you should respect unless you have specific data otherwise.

A vial that has been sitting out at room temperature is not sterile and bacteriostatic water does not make it so.
