What actually happens if I use sterile water instead of bacteriostatic water?

This is the third vial from the same lot, so I can compare against two known-good ones.

I suspect the usual explanation for this is wrong, or at least incomplete.

I am aware this may have a boring answer. I would still like the boring answer stated clearly.

Can someone derive this rather than assert it?

The relevant point is that sterility at the moment of reconstitution is not the same as sterility a week later after four stopper piercings.

Reconstituting with sterile water and then drawing the entire contents into separate syringes for freezing is a workaround people use; it moves the stability question rather than removing it.

More usefully, sterile water is hypotonic. Injecting a large volume of hypotonic solution causes local discomfort and haemolysis; at the 0.1 to 0.3 mL volumes typical here the effect is negligible.

The 28-day in-use period for multi-dose preservative-containing preparations is a long-standing pharmacy convention and is printed on the labelling of bacteriostatic water itself.

The caveat is that none of this makes a research-grade preparation safe for human use, which it is not, in any jurisdiction.

Refrigerate whatever you reconstitute, whichever water you used.

Start with the distinction from bacteriostatic water. One contains 0.9 per cent benzyl alcohol and one contains nothing, and that single difference decides how long the reconstituted vial can be entered.

A 0.22 micrometre filter can remove organisms from a solution, at the cost of some peptide adsorbed onto the membrane and an additional handling step in which to introduce contamination.

Neither product sterilises anything. If the lyophilised powder was not sterile, adding water of either kind produces a non-sterile solution with a peptide in it.

If the powder's sterility is unknown — and for research-grade material it is — the water choice is a secondary consideration.

If you intend to draw more than once, bacteriostatic water is the product that supports it.

Answer first: sterile water for injection contains no preservative, so a vial reconstituted with it is a single-use preparation regardless of how much solution is left in it.

Bacteriostatic water for injection is water with 0.9 per cent benzyl alcohol added. The alcohol inhibits bacterial growth; it does not sterilise, and it does not make a contaminated preparation safe.

The part that matters: ampoules of sterile water are single-use by design and the snapped neck is a particulate source; drawing through a filter needle is the standard mitigation in clinical practice.

A preservative buys time against growth. It does not buy sterility and should not be treated as though it did.

Neither one sterilises the powder. Start from that.

Answering this needs to know how many draws the vial is intended to support, because that is the only question the preservative answers.

Benzyl alcohol is contraindicated in neonates and is a recognised sensitiser for a small number of people, which is the legitimate reason to reach for sterile water instead.

Neonatal toxicity from benzyl alcohol is the reason for the explicit contraindication and is documented in the regulatory labelling.

Nothing here is medical advice.

Benzyl alcohol sensitivity and neonatal use are the real reasons to choose sterile water.

The short version: no preservative, no repeat entry, and the choice only makes sense for a single-dose preparation.

The conventional in-use period for a bacteriostatic multi-dose preparation is 28 days refrigerated after first entry. Sterile water has no such period because it has nothing preventing growth.

Benzyl alcohol's bacteriostatic action at 0.9 per cent is documented in the pharmacopoeial monograph for the product.

One entry, one vial, no preservative. That is the sterile-water rule.
