If I reconstitute a 15 mg vial with 1 mL of phosphate-buffered diluent, what concentration do I end up with?

The particulars: 15 mg · 1 mL · phosphate-buffered diluent.

I would like the arithmetic checked rather than the conclusion asserted.

I have deliberately not used an online calculator because I want to be able to check the result.

Concretely, the distinction that resolves most of these questions is that bacteriostatic water suppresses microbial growth and does not sterilise anything. It buys you a multiple-withdrawal presentation; it does not make an unsterile preparation sterile, and it does not substitute for technique.

On re-freezing something that thawed in transit: if it arrived as a lyophilised solid that warmed but never got wet, re-freezing costs you nothing except the thermal cycle.

More usefully, a 0.22 µm syringe filter will remove particulates and organisms, and it will also adsorb a fraction of your peptide.

The general principle — that peptides adsorb and denature at air–liquid and solid–liquid interfaces — is standard formulation science.

The caveat on all of this is that it assumes the vial contains what the label says.

The practical summary: fine gauge, gentle swirl, diluent down the wall, room temperature before drawing, and check the syringe scale.

To be exact about it, before anything else: a kitchen counter with an alcohol wipe is not an aseptic environment, and it is worth being honest about that rather than pretending the procedure is something it is not. What you are doing is reducing bioburden, not achieving sterility.

The order of operations matters: temperature first, then diluent measurement, then injection, then gentle dissolution, then labeling.

Number of stopper piercings matters less than the gauge doing the piercing.

Published data on syringe dead space in the context of injection-equipment programmes quantifies low-dead-space designs as retaining under 2 µL against 35 µL or more.

None of this is exotic. It is just the difference between doing it deliberately and doing it approximately.

Put another way, read the cake before you touch the vial. An intact, opaque, evenly distributed puck that sits proud of the vial base is what a good lyophilisation cycle produces. Anything else — collapse, melt-back at the stopper, a glassy film, a cake that has slumped to one side — is evidence about the cycle, the shipping, or both.

Photograph the vial against a matte black card with a single point light source off to one side, not with a flash from the front.

More usefully, do not use the same needle to pierce the stopper and to administer.

Coring of elastomeric closures is a well-characterised failure mode in the parenteral packaging literature.

Do the arithmetic twice, ideally with someone else doing it independently.

If the supplier documentation specifies a diluent, there is usually a reason, and if it specifies nothing, water for injection is the conservative default.

Room temperature before drawing is worth the ten minutes. Cold solution is more viscous, draws slower, and is more likely to pull a bubble past the plunger seal.

The 28-day beyond-use convention for a multiple-withdrawal preserved preparation derives from USP compounding chapters, which set it on microbiological risk rather than chemical stability.

I would flag the obvious failure mode: people get the concentration right, get the volume right, and then read the syringe against the wrong scale.

Check the barrel marking, not your memory of it.
